Subject: [PATCH] Widen entry form to fill available window width, capped at
 160ch

The New Entry / Edit Entry form was hard-capped at max-w-2xl (672px)
regardless of window size, wasting space on wider screens. Let it grow
with the viewport up to 160 columns instead, closes #23.
