Subject: [PATCH] chore: add portable and Claude plugin manifests

Publish root plugin.json (Agent Plugins 1.0.0) and .claude-plugin/plugin.json
so the existing sshx skill can be listed in Cursor Marketplace and the Claude
Plugin Directory. Version tracks the latest release, v0.19.0.
