Summary

A full review of the repository: the Bash CLI, the macOS menu-bar app, the Raycast extension, scripts/, tests, docs, workflows and packaging. Seven read-only reviewers each took one area; I reproduced every finding below before fixing it. Each code fix has a regression test, and every new CLI test was checked to fail against the CLI on main.

Nothing here reads, copies, prints or compares auth.json tokens or ChatGPT cookies. The scope contract in AGENTS.md is unchanged.

Fixed findings

Security

# Finding How it was confirmed Fix
S1 Terminal launches were not fish-safe (menu-bar app and Raycast). AppleScript's quoted form of uses POSIX quoting, but fish reads \' and \\ inside single quotes as escapes. Under fish, a folder named a\';echo INJECTED;# broke out of the quotes and ran echo INJECTED. A folder ending in \ failed to parse. The CLI accepts such folders as workspaces. Ran the old and new quoting through /bin/sh and fish 4.8. The old quoting printed INJECTED under fish; the new quoting prints the literal name in both. Both apps now build the command in code, writing quotes and backslashes outside the single quotes. The finished command goes to osascript as one argument, so the AppleScript is just do script (item 1 of argv). Tests in Swift and TypeScript run real commands in sh, zsh, bash and fish (when installed) against folder names containing \', \\ and a trailing \.
S2 clone-config copied common secrets. It copied files containing BRAVE_KEY, GITHUB_PAT, OPENAI_KEY, --key, key =, URL credentials (postgres://u:pw@…), sk-… tokens and Sentry DSNs. Its help and docs also said it checked "key names" when it actually matches text anywhere in the file. A table of 17 config lines run through the CLI on main and on this branch: only token = … was refused before. The check now also matches _key/-key/_pat suffixes, key/dsn as whole words, credentials in URLs and common token prefixes. Ordinary config (model, keybindings, MCP args) is still copied. The message and docs now say "sensitive-looking text".
S3 set-npm-token.sh put the token in env's argument list (env -i … NODE_AUTH_TOKEN=$token npm), where ps and exec audit logs can see it. The script's header promises this never happens. The existing test missed it because it logged only npm and gh arguments, and npm's own log line never ran because env -i drops TEST_LOG. Added an env stand-in that logs its arguments; the updated test fails on main with "the token appeared in a command argument list". printf (a builtin) sends the token on stdin to /bin/sh -c 'read …; export NODE_AUTH_TOKEN; exec npm …' under the same env -i environment.

CLI (bin/codex-profile)

# Finding How it was confirmed Fix
C1 status --json printed invalid JSON whenever a profile was in the error state, including the common case of a fresh machine without Codex CLI. json_emit_status_profiles_array turned errexit back on before its own nonzero return, so the closing } was never printed. Fake codex failing for one profile: Python's json.load failed on main; valid on this branch. Calls that can fail now run as conditions (… || status=$?) instead of toggling set +e/set -e.
C2 status --json ../evil exited 0 and reported a profile named ../evil with an empty home. Repro on main. The positional profile is validated.
C3 Plain status with no Codex CLI printed the error for every profile, then ran env "" login status (exit 127). Repro on main. It stops once with exit 1.
C4 launcher create reported success after a failed build step. The build ran in if ! ( … ), which disables errexit, so a non-executable copy produced "Created launcher" with exec '' app 'work' and recorded the launcher as managed. Reviewer repro, rerun on this branch. Every build step is checked explicitly.
C5 launcher create --force could not change only the letter case of a name on a case-insensitive volume. The rollback left the bundle renamed while the state kept the old name. Reviewer repro. When the old and new paths are the same file (-ef), the bundle is staged once. The test runs only on case-insensitive volumes.
C6 The default launcher name was invalid for some valid profile names, such as names ending in -, . or _, or names over about 72 characters. setup aborted if you accepted the launcher step. launcher create work- on main. The default name is truncated to 80 characters and trailing punctuation is trimmed.
C7 An interrupt during launcher create left .codex-profile-launcher.* in ~/Applications. Reviewer repro, using TERM sent mid-build. A trap removes the temporary bundle while it is being built.
C8 remove Work bypassed its safety checks on a case-insensitive volume. It deleted ~/.codex-work even while w2 linked its config, and left work's bindings behind. Reviewer repro on APFS. remove refuses a spelling that differs from the stored one and names the stored spelling.
C9 An exported CDPATH broke relative workspace paths for workspace bind/status, app and run --app. cd printed the directory, so the captured path contained a newline (or pointed at another folder). A bare - meant $OLDPWD. Repro with CDPATH=.:$HOME. unset CDPATH at startup, and relative paths get a ./ prefix before cd.
C10 A deleted workspace couldn't be unbound with the path used to bind it when a parent folder was a symlink (/tmp, a symlinked ~/Dev). Help promises unbinding works for folders that no longer exist. Reviewer repro. A surviving parent is resolved the same way bind resolved it.
C11 doctor --json exited with only a bash error when the guard file or registry was unreadable. chmod 000 guard-mode, then doctor --json. Diagnostics report registry_valid: false. workspace commands now give a clear "Cannot read" error.
C12 Stale-lock reclaim race. Two waiters that saw the same dead PID could both reclaim it, and the slower one deleted the lock the faster one had just taken. Both then rewrote workspaces.tsv, and a binding was lost. Reviewer repro with delaying shims. The new test, where a ps stand-in plays the other waiter, fails on main. A waiter claims a stale owner file by renaming it, which only one waiter can do. A file that turns out to name a live owner is put back. A lock with no owner now says how to recover.
C13 app-instance (and app --instance) with no profile failed with a usage error instead of opening the picker, as its help says it should. script TTY repro. The picker opens after option parsing. Added to the interactive cancellation matrix.
C14 A workspace starting with - reached open as an option (app work -- -h). open(1) has no --. Fake open logged files=-h. Relative workspaces are passed as ./….
C15 env/use accepted a symlinked profile home, for example ~/.codex-work -> ~/.codex, which every other command refuses. The shell then used another profile's auth.json. Reviewer repro. env refuses it.
C16 The Bash/Zsh --prompt indicator kept stale prefixes. Tools that save and restore PS1 (Python venv) left [codex:personal] [codex:work] $ or [codex:work] with no profile active. venv-style save/restore repro in bash and zsh. The hook removes every valid [codex:<profile>] indicator, not only a leading one.
C17 upgrade --prefix .local installed into the upgrade cache and reported the old binary as installed. It also dirtied the cache, so the next upgrade failed. Reviewer repro with a local git fixture. Relative prefixes are anchored to the current directory.
C18 upgrade --ref bin installed HEAD. git checkout bin restored a path and exited 0. Reviewer repro. The ref must resolve to a commit, and checkout uses --.
C19 Zsh completions saved as _codex-profile, as USAGE.md says, worked on the first Tab, then on every other one. Each later call re-registered compdef instead of completing. Called the autoloaded function three times in one zsh: the output was client, empty, empty on main. The body also completes when running as a function (*:shfunc). Sourcing at top level still registers with compsys; both are tested.
C20 doctor completions omitted -j. Generated completions. Added -j to Bash/Zsh and fish, and tested it by exact line.

macOS menu-bar app

# Finding How it was confirmed Fix
M1 Turning on low-quota alerts could notify immediately. When another profile's fresh reading arrived first, usageDidChange used a still-refreshing profile's stale reading as its baseline. That contradicts DESIGN.md. Reviewer probe compiled against the real sources. The new two-profile test fails without the fix. Readings still refreshing are left out of observation; they keep their earlier baseline. The test-only loadPreview now sets readingUsage before usage, matching the real refresh order, because usage's didSet reports immediately.
M2 Every test run left empty plists in the real ~/Library/Preferences (about 20 per run; about 650 on this Mac). removePersistentDomain leaves a 42-byte file, HOME doesn't redirect cfprefsd, and deleting the file in-process loses a race with cfprefsd's late flush. Counted files before and after a run. Suites are named with a per-run PROFILE_TEST_DEFAULTS_RUN tag, and menu-app-test.sh removes exactly that run's plists on exit. The file count is unchanged after a run. Older leftovers were not touched.

Raycast

# Finding How it was confirmed Fix
R1 A usage timeout was reported as "exited with status 1". command_usage traps TERM and exits 1, so error.signal is null. Reviewer ran run() against the real CLI with a silent fake codex. timedOut is killed && !aborted.
R2 A stale profile filter could hide every binding. The dropdown disappears with only one profile, so the filter couldn't be cleared. Code reading. Not run in Raycast. Falls back to All Profiles when the filtered profile has no bindings.
R3 Copied commands assumed codex-profile on PATH. That fails for menu-bar-only installs, which the README now leads with. Code reading. Copied bind, unbind, setup and init commands use the resolved CLI path.

Release, packaging and outreach scripts

# Finding How it was confirmed Fix
P1 The AUR clean build never failed on namcap errors. errexit ignores ! grep …. Reproduced: the script continued past a matching E: line. Uses an explicit if … exit 1. The test also forbids a bare ! gate in verify.sh.
P2 The Pages check could fail a deployed site. curl | grep >/dev/null under pipefail: GNU grep stops at the first match, and the marker sits past the pipe buffer, so curl can exit 23. Reasoning plus page-size measurement. GNU grep isn't installed here, so not reproduced. Reads the page into a variable, then greps it. No deterministic test, since macOS grep reads everything.
P3 The outreach tracker's HTTPS check was unanchored, so it accepted http://127.0.0.1.attacker.example/… and would send the JWT in the clear. New test fails on main. Parses with new URL() and allows HTTP only for exactly 127.0.0.1.
P4 list --owned false listed owned targets. New test fails on main. Uses asBool.

Docs and drift

# Finding Fix
D1 The AGENTS.md and CONTRIBUTING.md sync rule names "the README command reference", which no longer exists; USAGE.md holds it. Names USAGE.md and per-command help.
D2 SECURITY.md's network model omits the GitHub Releases API call that upgrade and upgrade --dry-run make. It also says "scripts" skip the update check, when the only gate is whether stdout is a terminal. Corrected in SECURITY.md and USAGE.md.
D3 USAGE.md says plain doctor is "informational", but it passes through a failing codex login status. Documented the actual exit status.
D4 llms.txt says plain list is "tabular" (it prints one name per line) and omits remove, path, logs, clone-config and version. Fixed, and added a Profile maintenance section.
D5 The docs say a launcher "calls codex-profile app <profile>". It actually runs the absolute path recorded at creation, so moving the binary breaks it. USAGE.md and llms.txt say so and tell you to recreate it with --force.
D6 README says setup's "Optional extras default to no", but sign-in defaults to yes and the launcher step isn't mentioned. Corrected.
D7 help usage omits that a missing CLI also returns nonzero. help remove omits the managed-launcher refusal. Top-level help omits CODEX_PROFILE_UPDATE_INTERVAL/CACHE/URL. Added, within 80 columns.
D8 The help test skipped usage, and USAGE.md's tested-platform list omitted it. Added.
D9 docs/welcome.svg was shipped in the npm package and required by a stale GEO-test assertion, though nothing references it. Removed from files and the test.
D10 A rerun after main moves past a pushed tag is refused (see "Not fixed"). CONTRIBUTING now says not to merge to main until a release finishes.

Findings not fixed

  • Resuming a release after main moves (verify-state.sh/verify-source.sh). This gate is deliberate: CONTRIBUTING says stale verification must not authorize a release once main or the tag moves. Allowing "tag at verified commit, ancestor of main" would loosen a safety gate, so that's your call. I documented the constraint instead.

  • usage reply parsing and app-server cleanup:

    • Quadratic parsing of a single huge reply line.
    • Helper processes of codex app-server not being signalled.
    • Leading-zero numbers producing invalid JSON.

All three need a non-conformant or unknown app-server. Replies are already capped at 1 MiB, and reviewers confirmed no tokens leak and cleanup works on interrupt.

  • Raw update-check version printed to the terminal. This needs a compromised registry or HTTPS interception, and npm enforces semver versions.

  • A pre-release on npm latest would be announced. The release pipeline never publishes pre-releases to latest.

  • Other credential variables reach the Desktop app. OPENAI_API_KEY and CODEX_API_KEY reach open. It's unverified whether ChatGPT Desktop uses them, and refusing them would break unrelated workflows.

  • The desktop log is replaced on each launch while a running window might keep writing to the old file. This depends on Electron second-instance behaviour I couldn't test without launching the app.

  • Changing CODEX_PROFILE_LAUNCHER_ROOT strands a recorded launcher. This is intentional strict state validation.

  • "Limit reached" at 99.5% used. All three clients round the same way.

  • Menu app: unverified or intended behaviours.

    • A failed usage read clears the low badge. This is intended per DESIGN.md: missing data stays unavailable.
    • A reassignment racing a refresh corrects itself on the next refresh.
    • Ordinary CLI calls have no time limit. In practice the CLI's 30-second lock bounds them.
  • Raycast/menu Terminal details.

    • A possible second window when Terminal isn't running is unverified and cosmetic.
    • tcsh ! history expansion and nushell's lack of && affect shells the project doesn't support.
  • Release robustness items I couldn't confirm.

    • A publish-npm.sh rerun refusing its own tarball after a toolchain change.
    • GitHub API rate limits in the installer check.
    • Tracker paging that stops on a short page.
  • Website hero terminal shows illustrative output. It's marketing art, not a command reference.

  • CHANGELOG 1.3.0 entry claims usage JSON states mirror status --json. I left the historical entry as shipped.

  • scripts/macos/render-menu-preview.sh leaves one plist per manual run. Sweeping right after its only binary exits would race cfprefsd's flush, and it's a rarely run dev tool.

  • Bash completion of paths containing : or =. Not reproduced.

Testing

  • make check passes: syntax, every behaviour suite, ShellCheck, and the native menu-app suite on macOS.
  • Raycast, inside raycast/: npm run typecheck, npm test (54/54), npx prettier --check . and npx eslint src test all pass.
  • I copied this branch's test/ next to main's bin/codex-profile and ran the changed CLI suites. status-json, launcher, profiles, workspace, desktop, shell, prompt, upgrade, completions and interactive all fail there; help-test only gained usage coverage.
  • The new npm-token test fails against main's set-npm-token.sh.
  • ~/Library/Preferences file count is unchanged after test/macos/menu-app-test.sh (1379 before and after).

Scope and compatibility

  • Affected scope: both. Codex CLI / CODEX_HOME, named Desktop profile launches (workspace argument), and the companions' Terminal launches.
  • Compatibility impact:
    • env/use now refuse a symlinked profile home.
    • clone-config refuses more files.
    • remove <Name> refuses a differently cased spelling on case-insensitive volumes.
    • upgrade --ref refuses names that aren't commits.
    • The npm tarball drops docs/welcome.svg.
    • Every other change makes a broken case work or report correctly.

Checklist

  • I ran make check; if the environment lacks ShellCheck, I ran
make test and documented the missing lint result instead.
  • I did not add code that reads, copies, prints, parses, uploads, compares,
or migrates auth tokens or ChatGPT cookies.
  • Desktop changes preserve the stock app default session and use the
    original signed app bundle.
  • I did not claim that CLI and Desktop account equality can be inspected or verified.
  • I updated documentation for user-facing behavior changes.

- status --json stays valid when a profile fails or the CLI is missing,
validates names, and plain status stops once without a CLI.
- launcher create checks every build step, fits default names to any
profile, handles case-only renames and cleans up on interrupt.
- remove refuses a differently cased spelling of a stored profile.
- unset CDPATH; anchor relative workspace paths; unbind resolves a
surviving parent; app passes ./-dash workspaces to open; app-instance
without a profile opens the picker.
- stale-lock reclaim claims the owner file by rename; doctor --json
reports unreadable state; env refuses symlinked homes.
- prompt hook drops stale indicators; upgrade anchors relative prefixes
and refuses non-commit refs; zsh completion works under any autoload
  name; doctor completes -j; clone-config refuses more secret shapes;
  help lists update-check variables.

- The menu app and Raycast build the Terminal command with quoting that
sh, bash, zsh and fish read alike, and pass it as one osascript
argument; quoted form of let a folder name escape quotes under fish.
- Low-quota alerts leave still-refreshing readings out of observation,
so another profile's reading cannot make a stale baseline.
- Native tests tag their defaults suites per run and the test script
  removes those plists from ~/Library/Preferences.
- Raycast reports usage timeouts as timeouts, resets a filter whose
profile lost its bindings, and copies commands with the resolved CLI.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
- set-npm-token.sh sends the token to npm on stdin instead of env's
argument list; the test now logs env's arguments.
- The AUR clean build stops on namcap errors (errexit ignores `!`).
- The Pages check reads the whole site before grepping it.
- The outreach tracker allows plain HTTP only for exactly 127.0.0.1,
and list --owned false lists unowned targets.

Co-Authored-By: Claude Opus 5.5 <noreply@anthropic.com>
- Name the USAGE.md command reference in the sync rules and note that
main must not move during a release.
- Complete the network model (upgrade's GitHub Releases request; the
update check follows whether output is a terminal).
- Describe clone-config's text matching, setup defaults, plain doctor
and usage exit status, list output and launcher targets accurately,
and add maintenance commands to llms.txt.
- Stop shipping the unused docs/welcome.svg in the npm package.
- Record the review's fixes under Unreleased.

Comment thread bin/codex-profile
Comment thread raycast/src/open-workspace.tsx
- remove no longer refuses a profile because a symlinked alias home
  (for example .codex-alias -> .codex-work) resolves to the same folder;
only real directories can be another spelling of it.
- Raycast Open Workspace forgets a profile filter once a finished load
confirms the profile has no bindings, so a later binding cannot
  silently reapply it.
