perf(inject): per-recipient session registry + caveman lessons (0.1.108) - #118

…ections Claude Code hook injections are delivered in full: blocks are chunked under 9 500 chars so no 2 KB preview truncation, the loader copy owns each block, true duplicates are trimmed and the per-prompt recall is kept. The registry is dropped on SessionEnd; pre-tool lessons fire once per session/sub-agent. codex/cursor/kimi/gemini output is byte-identical.
The Stop reminder and the per-prompt lessons suffix now ask for telegraphic bullets that never drop not/never/no/only and keep commands, paths and numbers verbatim. MEMORY/LESSON.md compacted to caveman style, triggers unchanged.
