docs: record the guide framing fix under Unreleased - #213

The 0.19.2 release consumed the Unreleased block, so #212 merged with nothing recording it. Restores the section and describes the fix, so the next release does not have to reconstruct what landed from the commit log. Co-Authored-By: Claude Opus 5 <noreply@anthropic.com>
