The builder now compiles to ClickHouse and Postgres, so the package becomes @maple-dev/effect-orm and the repo MapleTechLabs/effect-orm.

• Imports and error _tag prefixes move to @maple-dev/effect-orm/... (breaking for code matching tag strings).
• Live-test variables are now EFFECT_ORM_CLICKHOUSE_URL / _USER / _PASSWORD; CI updated.
• Docs homepage is unchanged (effect-clickhouse.maple.dev) until the docs site moves.

Verified: typecheck, bun run test (358 passed, live-CH suites skipped locally), test:package.
