docs: add the screenshot with approval steps drawn in by hand - #9

The article shows two versions with the same loop shape: approval drawn
in as your own steps ("read and fix the design", "decide whether to add
it to the policy"), then the same approval written down (a sign-off
field, a PR merge). where-am-i-4.png is the second; this is the first,
so the image matches the steps named in the text.
