  • Generic reviewers catch the filesystem form of time-of-check to time-of-use races and miss the database, authorization, cache, and distributed-store forms, which are the ones that cost money and data in our systems.
  • Race findings without a named interleaver and a named atomic fix get dismissed as theoretical, so the skill forces both into every finding and requires a named serializer before a pair can be marked safe.
  • The expected-version and revision vocabulary already lives in the eventmodeling and nats plugins; this skill reuses it rather than restating it.

PR Summary

Low Risk
Documentation and plugin metadata only; no changes to production services, auth, or data paths.

Overview
Introduces the trogonstack-concurrency plugin (v0.0.1) and wires it into the Claude/Cursor marketplaces and release-please so it ships like the other TrogonStack skills.

The substantive addition is the concurrency-find-toctou skill: a review playbook for check-then-act races across filesystems, SQL, authorization, caches, in-process/async code, and distributed stores (NATS KV/JetStream, event-store versions). It defines seven principles (reads are stale, owner decides, condition in the write, named serializer, lock discipline, retry around conditional writes, idempotence), a mandatory finding template (check, act, window, interleaver, consequence, fix), and a fix catalog plus layered references (surfaces → Postgres/MySQL/SQLite/Redis/NATS → per-library spellings for TypeScript, Rust, Elixir, Go, Python, C).

No application runtime code changes—only agent guidance and reference markdown.

Walkthrough

Adds and registers a concurrency plugin with a TOCTOU review skill. The skill covers race identification, finding reports, and reference guidance for filesystem, runtime, authorization, cache, database, and distributed-store patterns across several languages.

Make payments.applyDiscount idempotent across ambiguous retries.

The conditional coupon claim does not deduplicate payments.applyDiscount. If the provider applies the discount and the request times out or crashes before the response, a retry claims another use and applies the discount again. Pass a deterministic redemption key that the provider enforces, or persist a redemption/outbox record and process it idempotently. Include this post-effect ambiguity in Residual, not only a crash before the discount.

Preserve the cache TTL in the conditional write.

Do not reject correctly fenced distributed coordination.

The final sentence says coordination in the caller helps only when every writer is in one process. A distributed lock can serialize writers across processes when every writer uses the same lock and the owner verifies a fencing token. Name the duplicate-window bound in the outbox row.

The row recommends reclaim after claim expiry but omits the stream's duplicate-window bound. The linked surface page states that bound. Add it to the row:

📝 Proposed catalog wording
-| Outbox | read unsent, publish, mark sent | Condition | claim rows with an expiring `UPDATE ... WHERE claimed_until < now() RETURNING`, publish with `Nats-Msg-Id` = row id, reclaim after expiry | [distributed](../surfaces/distributed.md) |
+| Outbox | read unsent, publish, mark sent | Condition | claim rows with an expiring `UPDATE ... WHERE claimed_until < now() RETURNING`, publish with `Nats-Msg-Id` = row id, reclaim after expiry within the stream's duplicate window; consumers dedupe on the message id | [distributed](../surfaces/distributed.md) |

Nats-Msg-Id absorbs a republish only inside the stream's duplicate window. A reclaim after that window stores the message twice. The crash alone does not duplicate the message.

Make the Scan example valid Go, or label it as pseudocode.

Scan is variadic over destination pointers, not over a pointer to a slice. &sub.Fields... passes a *T to a ...any parameter, which does not compile. Use explicit destinations, such as Scan(&sub.ID, &sub.Plan), or build a []any of pointers and pass that slice with ....

Scope the lock claim to the contested key.

A missing-row SELECT ... FOR UPDATE does not lock a row, so it does not serialize two concurrent inserts. PostgreSQL locking clauses apply to rows obtained by the query. (postgresql.org)

Include the missing-row case in the changes() result.

A zero result means that the UPDATE changed no row. It can mean that the row is absent as well as that the status no longer matched. Correct the LAST_INSERT_ID() round-trip claim.

The example contains an INSERT and a separate SELECT, so it requires two round trips unless the client explicitly uses a multi-statement request. Both statements must also use the same MySQL connection because LAST_INSERT_ID() is connection-scoped. (dev.mysql.com)

Show the driver’s insert-id result from the INSERT, or remove the one-round-trip claim.

Version-gate the Prisma where capability statement.

Current Prisma permits update to include additional non-unique fields when at least one unique field is present. updateMany remains useful when the caller needs a count, but it is not the only Prisma API that accepts a non-unique filter.

If this reference targets a pre-4.5 Prisma client, state that target explicitly. Otherwise, remove the “only” wording.

Suggested fix
-`update` only accepts unique fields in `where`, so it cannot carry a status or version condition. `updateMany` can, and returns `count`.
+`update` requires at least one unique field in `where`, and current Prisma can also accept additional non-unique fields such as `status` or `version`. Use `updateMany` when you need a `count`.
...
-`prisma.order.update({ where: { id } })` after a `findUnique` that checked the status is the race; `updateMany` is the only Prisma call that lets a non-unique column into `where`.
+`prisma.order.update({ where: { id } })` after a `findUnique` that checked the status is the race; use `updateMany` when you need a `count` from a non-unique condition.

Clarify the two update_counter modes.

The clamped tuple {2, 1, limit, limit} can return limit to multiple callers, so it cannot identify a winner. The unbounded {2, 1} form returns distinct values. When all callers use this rollback protocol, callers that receive a value greater than limit can decrement once; interleaving does not inherently undo the successful increment. Clarify this distinction.

Suggested fix
-A limit cannot be enforced inside `update_counter`; it only clamps with a threshold tuple (`{2, 1, limit, limit}`), so a caller that must know whether it won checks the returned value against the limit and decrements on loss, or uses `select_replace` below.
+A threshold tuple (`{2, 1, limit, limit}`) clamps overshoots to `limit`, so it cannot identify a winner. With an unbounded `{2, 1}` update, each atomic increment returns a distinct value; when the counter starts below `limit`, the caller that receives `limit` wins and callers that receive a value greater than `limit` can each decrement once. Use `select_replace` below or a serializer when this rollback protocol is not guaranteed.

Qualify the arbiter rule for DO UPDATE.

Line 29 is generic and also covers the later DO NOTHING case, where exclusion constraints are valid. However, the preceding example uses DO UPDATE, which requires a non-deferrable unique index or unique constraint. Qualify the sentence instead of removing exclusion constraints.

Suggested wording
-`ON CONFLICT` needs an arbiter: a unique index, a unique constraint, or an exclusion constraint matching the conflict target. Without one the statement is rejected at parse time, which is a useful property; the arbiter cannot be forgotten. `RETURNING` fires only for rows the statement inserted or updated, so `DO NOTHING` returns no row on conflict and the identity needs a follow-up `SELECT`, which is safe because the row now exists.
+`ON CONFLICT DO UPDATE` needs a non-deferrable unique index or unique constraint matching the conflict target; exclusion constraints are not supported as arbiters for this action. `DO NOTHING` can also handle exclusion-constraint conflicts through `ON CONSTRAINT` or with no conflict target.
