heartbeat stops rewriting the immortal agent catalog. reconnect layer backs off and stops blocking on journal pipes.

🐛 Bug Fixes

• split agent_presence (utc-day epoch) from immortal agent catalog; heartbeats upsert presence only
• prod presence writes at most hourly (ENVIRONMENT=production); connect/disconnect still update server.status
• alarm clears stale online when agent socket is gone
• supervisor: exponential reconnect backoff (1s→60s), rate-limited health logs, nonblocking stdout

🚀 Features

• fleet / servers / detail read lastSeenAt from presence map
• revoke deletes presence row
