docs(plans): record Wave B merge as PR #668

Plan 22 Wave B completion

Implements the remaining Wave B items from plans/22-codebase-improvement-2026-10.md
(Python quality relevance, Rust routing-state persistence + provider parity,
web history/is-url/provider fixes), with tests updated alongside.

• Python: query-relevance signal in quality scoring; MIN_CHARS aligned with
  the quality threshold so accepted content is not re-flagged as too short.
• Rust CLI: routing memory / negative cache / circuit breakers persist across runs;
  synthesis falls back when the LLM call errors; llms_txt keeps the port;
  link validator retries with GET on 405/501; compaction truncates by chars;
  case-insensitive URL extension detection.
• Web: history restore shows the stored result instead of re-running; shared
  isUrl helper; correct provider count.

Verification: pytest -m "not live", cargo test (cli), web vitest + lint +
typecheck + build + Playwright (desktop/mobile/tablet) and ./scripts/quality_gate.sh.

Opened and merged by the temporary .github/workflows/wave-b-automerge.yml
workflow (dispatches the required CI workflows, waits for the required checks,
squash-merges). The workflow itself lives only on the throwaway
ci/wave-b-automerge branch and is not part of this change set.
