Add engineering loop to default workflow

What changed

• show the core Maker → Verification → Review → Judge cycle as an Iteration Loop in the default Studio template
• return only the loop nodes to Maker when Judge requests rework
• continue to approval and Delivery only after Judge passes
• label the two paths as ITERATE · REWORK and PASS · CONTINUE
• bump the package to 1.9.2
