Goal

REVIEW.md states the anti-loop rules from plans#76 so reviews cannot loop.

What changed and why

Severity: blocker/major only when a Done criterion, test, correctness or vision principle breaks, and the finding names it (the old "duplicates existing code" and "Zig-standard break" majors fold into this). New Rounds section: evidence before review, VERIFY checks only the RULING's must-fix list, at most two fix rounds then one PM decision. Points to AGENT_WORK.md for stages instead of repeating them; consistent with it (it already says the same two-round rule).

How it was verified

Docs only: scripts/check-fmt.sh passes. Read against docs/AGENT_WORK.md (PR #822 is merged) for contradictions: none.
