Fail the panic-attack gate when the findings payload is unusable.

Lines 56-58 convert missing scanner output to []. Lines 66-68 only warn for other invalid payloads. If the scanner fails before it writes findings, or writes malformed JSON, the counts become zero and the critical-finding gate succeeds. Reject an empty or non-array payload with a non-zero exit instead of treating it as no findings.
