When a finding already has exact evidence from an added line of code, an incorrect extra source location supplied by the model can still leave the review failed. Adjudication now uses the verified evidence from that added line as the cause instead of validating the redundant reference. Findings on unchanged lines still need a valid reference to changed code, and long added lines retain a valid explicit reference when they cannot supply a bounded exact excerpt.
