Reviews fail when an otherwise valid finding carries a causal reference that does not match changed source. Adjudication accepts a displayed diff-row reference bound to the source hash and resolves the exact file location itself. Resolving a diff-row reference requires visible, unambiguous changed source; the existing exact-source reference format remains supported.
