Bump all workspace crates to 0.13.0 and add the v0.13.0 changelog entry. Also fix the `[v0.10.0]` compare link, which pointed at `logfire-v0.*` tags that don't exist for 0.10 onwards, and add the missing `[v0.11.0]` and `[v0.12.0]` link definitions.
