Merges upstream jmoiron/sqlx#805 by Xudong Cai with its original commit: a pointer to a slice batch-inserts instead of panicking in reflectx. Fixes upstream #577 and #729. A follow-up commit completes the pointer-to-map case, which the PR still left panicking, by dereferencing in
bindNamedMapperandconvertMapStringInterface. The test inserts through*[]struct,*mapand*[]map.Local run with Postgres 16, MySQL 8.4 and SQLite: 88 pass, 3 skip, 0 fail.
