Investigate and remove superseded blanket switches

The table below lists the current blanket switches. Modify so that only iblanket switch is required.

Removed from fispact and costs but:

• still use blktype in costs_step, initial and stellarator,
• still use blktmodel in availability, costs_step, stellarator_fwbs and stellarator.

Having looked at the current state of the code there is a lot of interplay for all of the availability models and the stellarator models, of which the dependencies need to be fully understood. Since this issue is purely syntactic it may be best to tackle when more of the pythonisation has occurred and or when the blanket models are reviewed and fully understood.

@chris-ashe Started this off by changing blktmodel == 0 to i_blanket_type == 0 and making a few other simplifications. I can't work out what "simple blanket model" actually is: I need to understand this before continuing. The stellarator logic is particularly difficult to understand.
