feat(chat): rework the top-level turn fold header - #1817

The turn header now reads as a hairline row: status, recorded duration and tool count stay in one left-aligned run, with the disclosure caret trailing the metadata instead of sitting at the far edge of the transcript. Hovering only strengthens the text colour, and the issue count is no longer repeated at this level because the status already names a failure. Nested activity-group headers and their hover fill stay unchanged so the transcript hierarchy below the turn fold is not flattened.
The top-level row no longer bleeds into the gutter, so its fill starts at the prose edge while nested group and run fills keep their 4px hover bleed. The shared 5px gap keeps the turn title on the same text line as the nested titles, and the transcript E2E probes the new content edge and full-width hairline.
The probe compared the header with the padded fixture root, so a correct full-width row could never match. Measure the header against the turn process that owns it instead.
